Spa microdermabrasion will deliver better results than an at home kit. Medical microdermabrasion will deliver even better results and Surgical microdermabrasion will give you the look most people desire.
Spa micro is relatively painless and the results will last for about 9 months. Medical micro is a little more uncomfortable but the results are double those of the Spa micro and it lasts longer.
Surgical micro is done in a hospital or clinic setting and sedation. Your face is "sanded" with a medical instrument by a surgeon. The results are spectacular but the cost is high and its painful and the recovery time is 6-8 weeks with repeated visits to the Dr.
I'm sure the at home kits will show improvement but since I've never used one I can't really comment. They are a starting point to judge if you want more improvement. Personally I'm going to try either the Oil of Olay or Anew because all of the products from these lines I've tried have been excellent. I have had Spa and Medical micro and the Medical grade is much better. Medical grade micro isn't available at all Spa's because it goes deeper and requires additonal certification.
